Our Care Management team provides personalized service to help you and your loved ones get the care, education, and resources you need to better manage your health.

Complex Case Management is a 90-day program serving members with complex chronic medical conditions that, if not well-managed, can lead to serious medical emergencies. Our team will work closely with you to evaluate and monitor your health to improve and better manage your conditions. We will coordinate health care services and communication between hospitals, clinics, and doctors to best support you and your family.
Transition of Care is a member-centered coaching intervention to empower members to better manage their health after hospital admission. Our team will work with you to make sure you are educated about your condition, have support and resources, and get the right care to feel better.

Using a multidisciplinary team approach, our HIV/AIDS program will help you find community resources and navigate health care to better manage your condition. Our team will help you take charge of your own health, and give you support to balance your medical needs with your work, home life, finances, and more.

EmblemHealth members diagnosed with HIV/AIDS qualify to enroll in the HIV/AIDS Case Management program. You can enroll or ask questions by calling our Case Management program at 800-447-0768, Monday through Friday, 9 a.m. to 5 p.m.
If you’re a GHI City of New York CBP member with diabetes, you have access to our nationally recognized A1chieve program. Our diabetes care and education specialists are registered nurses and dietitians to help you:
